Important Information for all Student Athletes

This memo explains the procedures that you will need to follow in order complete your required medical forms and insurance information in order to participate in athletics at Iona University. The forms must be completed online through ATS (our Electronic Medical Record system) before the day your team is scheduled for orthopedic screenings.

If you have not yet received instructions on ATS, please reach out to the athletic trainer assigned to your sport, or email Dave Zippo dzippo@iona.edu for step by step directions on how to navigate through ATS to complete paperwork. 

If forms are not completed, you will NOT be able to receive an orthopedic screening and thus not be able to participate in intercollegiate sports at Iona University, until all required forms are completed.

All student-athletes must log into iona2.atsusers.com in order to complete/review paperwork and requirements. Your log-in to this website will also be how you sign up for rehabilitation appointment times with your assigned athletic trainer.

Please contact athletic trainer Dave Zippo at dzippo@iona.edu for ATS directions, if you have not received them in your Iona email yet. You may also contact your sport’s athletic trainer or Dave Zippo if you are a returning student-athlete and have forgotten your Athlete ID and/or password.

The following forms below must be uploaded to ATS prior to your arrival to :

• Completed demographics section, which includes general information, insurance, and emergency contacts.
• CLEAR picture of your insurance card
• The following forms must be filled out by all student-athletes:
  • Medical Questionnaire
  • Injury & Illness Awareness Agreement
  • Drug Testing Consent Form
  • Concussion Facts & Concussion Statement
  • Student-Athlete Consent for Information Release
  • Out of Network Acknowledgment Form
• In addition, returning student-athletes must fill out Injury Disclosure form.
• All new student-athletes must submit their sickle-cell test results.
• 2025 Physical that is stamped by the office of the physician/doctor

If you are UNDER THE AGE OF 18, a parent/legal guardian must sign all required forms, and must look over the Iona University Sports Medicine Online Presentation.
